    Who needs to complete this form?

    • All families new to Bangor Township Schools must complete this form for each student - even if your student will NOT ride the bus. This ensures our records are accurate for every student in the district.
    • Any student who will ride the bus at any time during the 2026–2027 school year - every day, certain days, or only occasionally - must have a completed form on file.

    Please submit a separate form for each student. Forms are due by AUGUST 14, 2026. If your student needs transportation to/from school as their primary transportation, this form must be completed or your student will not be able to ride the bus for the 2026–2027 school year.

    ⚠️ Forms submitted after August 14 may result in a delay before bus service begins. Your student's routing cannot be guaranteed for the first day of school. Please submit as early as possible.
