As a prospective volunteer for the Bangor Township Schools, I understand that it is the school’s policy to secure a conviction criminal history information and/or to check references as part of their screening process using the information provided below. The Bangor Township School District is required to obtain complete criminal background information regarding employees and volunteers. Your complete and accurate response is required. Conviction may or may not disqualify you, depending on the nature of your offense. However, checking no when there has been an occurrence will result is an automatic denial.

Some volunteers may volunteer in more than one school. However, it is only necessary to fill out this form one time, as it will be kept on file in the district office and is valid for two school years.

Please complete this form only once per volunteer
Once your application is approved, it will be valid for all students in all Bangor Township Schools. Your approval will remain valid for two (2) years, after which you will need to reapply.

Do not complete multiple applications if you have more than one student enrolled. Submitting multiple forms may delay the processing of your application.
